In order to activate that option in your site, please follow these instructions below :

Create the note field

  1. From the admin page, click to “forms” then choose a specific form that you want to configure
  2. From the admin page, select “ add elements
  3. Choose textarea
  4. Edit the label as “ note” and adjust collum width

Map the note attribute into created field

  1. From the form page, click to “ settings
  2. Click to “ integration
  3. Select “ Shopify”
  4. Enable the checkbox “ Create Shopify customer
  5. Map note attribute into created field
  6. Save the form to update the change
